We all have moments when we consider what we want in life and how we wish to be cared for. This exercise will guide you through a series of questions, akin to crafting a personalised roadmap that highlights your preferences and choices for the future.

Are you ready to begin this empowering journey? Remember, there are no "right" or "wrong" answers here; it's all about expressing what resonates with you.

What are your current mental health diagnoses,if any

Have you had any treatments, therapies, or medications in the past that you found beneficial?

Were there any treatments, therapies, or medications you had adverse reactions to or wish to avoid?

Are there specific medical professionals or therapists you'd trust to consult regarding your care during a crisis?

How would you define a situation where you might be unable to make informed decisions about your mental health?

Who should we contact in case of an emergency? Please provide their names, relationships to you, and contact details.

Are there personal coping mechanisms or activities that help calm you during distress (e.g., certain music, breathing exercises)?

If hospitalisation is needed, do you have a preferred hospital or clinic?

How do you feel about treatments like electroconvulsive therapy (ECT) or similar interventions?
